Registration Forms
1. Dates
The school year at Jan Harvey's Music and Movement
for Children runs September through December (Term 1) and February
through May (Term 2).
Registration for Fall 2011 classes will take place at 9am on MONDAY,
APRIL 11th, 2011 and is on a first-come,
first-served basis. Please bring a filled-out registration
form with you for each child registering.
Registration is held on-site at 1580 Bernard
Avenue (see location). After registration
day, please contact Jan Harvey directly
to see where spaces may exist.
2. Fees for the 2010-2011 Year:
Registration Fee: $20 per child
per year, paid by cash or cheque at time of registration. Non-refundable.
Term Fees: The cost for each term
is $280.00. On registration
day, please provide a cheque for your first Fall installment ($80.00) dated
September 1, 2011. We are happy to provide two options for paying
the balance of fees:
1) Pay by the Term
In September, please provide two cheques:
- one dated October 1, 2011, for $200.00, covering the balance of the
first term, and
- one dated February 1, 2012 for $280.00 covering Term 2: February-May,
2012.
or,
2) Pay by the Month
On registration day (April 11, 2011), provide 7 postdated cheques for $70.00 each, dated on the first
day of October, November, December, February, March, April and
May.
3. Getting the Right Class
If the class you want is full on registration
day, don’t panic. Put your child’s name on your second
choice or on the wait list for your first choice. Things always change
in the fall and somehow everyone ends up getting just what they want.
Kindergarten parents: Jan will do her best to accommodate your children
no matter what changes are made in September.
4. Making
Up Missed Classes:
If
a class must be missed due to illness or other reasons, students
may attend a class suitable to their age and program during the
week of the missed class. Check with Jan Harvey for an appropriate
lesson to attend as a makeup class. No refund is given for missed
classes.
5. Withdrawal from the Program:
A month’s notice is required for withdrawal.
All cheques issued for fees following the expiry of the notice period
will be returned. If no notice is given, fees will be retained
to cover the notice period, and the balance of the fees will be returned.

Please print off and fill-in one registration
form for each child. Bring filled-in registration forms with you
on registration day. If you are contacting Jan Harvey after registration
day, please discuss class options with Jan first, and then mail
the form, along with payment, to Jan Harvey, 374 Park Avenue
, Kelowna BC, V1Y 5P8.
